Failure of new installed concealed WC cistern fitting causing water damage to ceiling below.

When did it happen?

What happened?
Water stains appearing in the Kitchen ceiling below the refitted Ensuite bathroom
What were the implications?
Water had to be turned off and only turned on when fittings were used like toilets and sinks.
What part did you play?
Photographed and informed fitter on Sunday after we established most likely was the new installation took photographs.
Is it resolved satisfactorily now?
In the process of being resolved. New Cistern to be fitted and ceiling to be repaired
How was it resolved?
Fitter appeared 8am and discovered the new concealed cistern had a crack. Removed cistern and turned of supply to WC.

House Builder in 1987 used punched out branch manholes in 3 locations without capping caused failure 20 years later.

Organized contractor

Yes
How was it resolved?
Contractor dug-up around 3 manholes and cap open branches.
